Home Health Aides Personal Care Aides
Provide personal care and assistance with activities of daily living. This can include bathing, dressing, meal preparation, laundry, light housekeeping and providing safety with patient mobility. These caregivers follow the instructions in the care plan and are supervised by a Registered Nurse.

Skilled Nursing by Registered and Licensed Practical Nurses
The Nurse may provide skilled nursing care, wound care, clinical assessments and supervise health care staff.
The Nurse may also provide such skilled services as: administration of injections, administration of intravenous fluids and medications.
Nurses are trained in providing education and training to all patients, families and caregivers regarding their illness, disease processes, diet medications and self care that assists the patient in reaching an optimal level of independent functioning and health maintenance.

Therapists
All Therapy services focus on improving the patient’s present level of functioning.
Physical Therapists
Provide activities to increase strength and endurance. These services may include: gait training using assistive devices, therapeutic exercises designed to strengthen and improve joint range of motion or to prevent joint contractures due to muscular disease, etc.
Occupational Therapists
Provide activities to regain skills and/or provide instruction on adaptations for the patient. Their services may include evaluation of the patient’s functional abilities, teaching activities of daily living to improve patient’s level of independence, etc.
Speech-Language Pathologists
Evaluate patient’s ability to swallow and communicate (hear, speak, understand and write) and develop plans to improve abilities. Speech therapy services provide training on improving oral motor control and functioning.
